Referral Process

  1. Send a referral/order via fax 912 438 6907

  2. Include any relevant medical records or evaluations (if available). Example: Hearing Screenings

  3. Our office will contact the patient to schedule an appointment.

  4. We provide evaluations reports and progress updates as appropriate.
